Supervision Agreement Form pdf: Students are required to print this supervision agreement form and review it with their site supervisor. After the student and site supervisor have reviewed and signed the supervision agreement form, they must return it to the faculty supervisor for review. This will need to be completed prior to the initial start date of the practicum and internship courses the student is enrolled in. Final submission of the form will need to be to the program coordinator.
